adl - assisted daily living asessment

 

We complete ADL assessments for clients usually funded by SIRA/ Workers Compensation that require an assessment to understand the need for   

  •  An injured worker’s ability to carry out self care and community access tasks.
  • Identify strategies to maximise and maintain a worker’s independence
  • Determining the need for equipment and therapeutic aids designed to compensate for any functional limitations by the use of equipment
  • Re-educate and train the worker in adaptive methods
  • Determine the need for modifications in the household
  • Recommend reasonable and necessary services such as domestic assistance (cleaning), lawn mowing, gardening, window cleaning, pet care and so on.

Functional Assessments are conducted by Occupational Therapists to understand the everyday challenges of a person with an injury or disability.  These assessments are commonly required to assess the capacity of the client's so that appropriate supports and/ or services can be put into place to maximise participation and quality of life.

Why are Functional Assessments needed?

  • To assess a person's functional capacity to complete an Access Request for the NDIS.
  • To comprehensively assess a clients needs and recommend funding required to meet those needs.
  • To understand why a client is experiencing challenges with their everyday tasks
  • To identify priorities so that funding can be used effectively.
  • To provide justification for further funding.

What do we assess in a Functional Assessment?

Our Occupational Therapists assess how a client performs their everyday tasks (Activities of daily Living) such as personal care (showering, dressing, grooming & toileting), domestic tasks (cleaning, meal preparation, laundry tasks) and community access tasks (money management, shopping, travel, employment/ volunteering).

The Occupational Therapist then assesses what support is needed for the client to reach their potential and/ or improve their life.

The Occupational Therapist uses a range of standardised and non standardised assessment tools to gather this information so that we have a complete understanding of our client and their needs.  The assessment tools may include measures of adaptive function (eg. Vineland III or the ABAS), quality of life (WHOQOL, or WHODAS) as well as specific task based assessments.  This assessment can take several hours to complete to ensure the assessment report provides information to capture the client's current and future needs.

Funding Options


We are registered with the NDIS for those who have access to NDIS funding otherwise sessions can be paid for privately or also rebated through Medicare. We also have access to other funding models, contact us to find out more.

Manual handling assessments

 We complete Manual Handling assessments for clients whom require assistance for transfers and/ or mobility. This can include

  • Assessment of transfers, potentially across multiple environments
  • Provision of summary document of the client's  Manual Handling needs
  • Detailed procedure of individual transfers if requested
  • Educational video with demonstration of transfers
  • Staff training and education on how to address individual client needs

aged care assessments

 Our occupational therapists are experienced completing assessments and recommendations through Department of Veteran's Affairs, the Commonwealth Home Support Programme (CHSP) and My Aged Care.

We also can complete assessments for people accessing Residential Aged Care.
